Please allow 1 hour for the assessment and ensure the CASA MRS questionnaire is completed ahead of time. Please bring all prescription eyewear to your appointment.

A deposit of $100 must be paid at the time of booking. This amount will be forfeited upon non-attendance or late cancellation (within 2 hours of booked appointment time).
